Learning objectives
By the end of this experience the learner can:
- Choose an appropriate subcutaneous site and rotate sites for repeated doses.
- Pinch the skin and insert at 45° or 90° as indicated.
- Inject without aspiration for insulin and heparin.
- Recognise lipohypertrophy and bruising as reasons to change site.

Procedure steps
- Confirm the medication, dose and site rotation record.
- Clean the site and pinch a fold of subcutaneous tissue.
- Insert at the correct angle and inject slowly.
- Withdraw, apply light pressure without rubbing and dispose of the sharp.
- Document the site used for the next rotation.
Assessment
Angle selection, site rotation and needle handling are scored, with feedback on bruising-prone technique.
Equipment and setting
Standalone VR headset with hand controllers, full-scale virtual patient and equipment, instructor screen mirroring, Arabic and English interface, offline operation on the campus network.
