This experience walks the learner through the full pathway: checking the prescription and consent, two-person verification of the unit against the patient, preparing the blood administration set, starting the transfusion slowly, monitoring vital signs at the required intervals and recognising and managing a transfusion reaction.

Learning objectives

By the end of this experience the learner can:

Verify the unit against the prescription, the patient's wristband and the compatibility label with a second checker.
Prime and connect a blood administration set with an in-line filter.
Start at the correct rate and monitor vital signs at 0, 15 minutes and hourly.
Recognise and manage a transfusion reaction: stop, maintain access, escalate and document.

Procedure steps

Confirm consent, prescription and baseline vital signs.
Collect the unit and perform the two-person bedside check.
Prime the blood giving set with saline and connect to the cannula.
Start slowly, stay with the patient for the first 15 minutes and repeat observations.
Complete the transfusion within four hours, flush the line and document the outcome.

Assessment

Any missed verification step ends the scenario with an explanation; monitoring intervals and the reaction response are scored.
